OTHER INFORMATION |
SPECIMENS |
Please hand in all specimens by 1.00pm so they can be transported to the laboratories.
CARERS |
This practice would like all our patients to know what support we offer to carers. If you are looking after a friend or relative who is ill or disabled, an adult or a child - you are a carer. On the other hand, you may have a friend or relative who regularly helps you with washing, dressing, preparing meals or running your home - they are your carer.
We would like to know who the carers are amongst our patients so that we can provide information, support and even arrange carer events. So please do let us know if you are a carer or have a carer.
CHAPERONES |
If you wish a chaperone to be in attendance during your consultation, please advise the receptionist who will arrange for a nurse to be present.
FREEDOM OF INFORMATION - PUBLICATION SCHEME |
The Freedom of Information Act 2000 obliges the practice to produce a publication scheme. A publication scheme is a guide to the 'classes' of information the practice intends to routinely make available.
THE HEALTH CARE TEAM |
We have a primary health care team working at the practice premises which aims for optimum primary care.
NON-NHS SERVICES |
We are happy to provide a range of non-NHS services. These include medical examinations which you may require, eg for insurance, pre-employment, elderly car drivers, special driving licence and reports on your fitness to undertake certain sports and travel. A fee will be payable at the time of the examination. Please ask for details at reception.
HERE TO HELP |
The doctors, receptionists and ancillary staff are here to help you and are always open to suggestions on how our services can be improved.
If you have an idea don't keep it to yourself, tell one of us!
Patients will be treated as individuals and partners in their healthcare, irrespective of their ethnic origin or religious or cultural beliefs.
The practice will offer patients advice and information on steps they can take to promote good health and avoid illness.
You have the right to see your health records, subject to limitations in the law. These will be kept confidential at all times.
Please read our practice booklet. It will help you to get the best out of the services we offer. Please ask us any questions if you are unsure of anything.
VIOLENT AND ABUSIVE BEHAVIOUR |
We aim to treat our patients with courtesy at all times, and expect our staff to be treated in the same way. We take any violent, abusive or threatening behaviour against any of our staff or patients seriously. If a patient is violent or abusive they will be warned to stop such behaviour. If the behaviour continues, we have the right to take action to have them removed, immediately if necessary, from our practice list.
